Becoming a Volunteer

Once you have been accepted into the Volunteer Program by submitting your application, attending an Informational Session, and interviewing with the Volunteer Services Department, you need to complete the following to join our team:

Step 1

Collect current vaccinations from your physician or Health Services office (Immunization Record Form). Do not get additional vaccinations until Employee Health reviews your health records.

Step 2

Review the Volunteer Orientation Manual and take a short quiz.

Step 3

Download and complete the following forms and turn them in during your 30-minute appointment for paperwork and vaccination review with our team:

  1. Volunteer Information Sheet
  2. Availability Sheet
  3. Reference Request
  4. If you are over 18, you will receive an email request from CERTIPHI, which will initiate a background check.
