Providers also coordinate care with other specialists as needed, to make certain you get the best treatment for chronic conditions such as diabetes, heart or lung disease, or depression. Our Physicians and Advanced Practice Providers are dedicated to providing the best possible care.
Our Staff
South County Primary Care is staffed by experienced, caring Physicians and Advanced Practice Providers (Nurse Practitioners and Physician Assistants) in both Family Medicine and Internal Medicine. Each offers patients seamless continuity of care if a specialist or hospitalization is required.
- Family Medicine: Providers that provide primary care for people of all ages - from newborns to the elderly.
- Internal Medicine: Providers that provide primary care for people aged 18 through the elderly.
When necessary, they will coordinate care with other medical and behavioral health specialists to make certain you get the best treatment options for your condition and lifestyle.
Patients whose primary care providers and specialty physicians are part of the South County Medical Group benefit from shared electronic health records, giving the team of interdisciplinary experts the ability to access patient information quickly and easily, ensuring accurate and immediate communication to give you the best possible care.
