When you are ready to leave the hospital, your case manager will meet with you and your family to ensure that you have all the information you will need including:
- Instructions from the medical staff
- Information about any medications you need to take
- Dietary restrictions
- Signs or symptoms that you should watch for
- Activities to avoid
- When you can return to work and resume normal activities
- Arrangements for any follow-up care you need, such as home health services.

Your Discharge Plan
Please have the family member or friend who will pick you up plan to arrive in the morning, so that the discharge instructions can be reviewed.
Your care team will develop a discharge plan based on your specific medical and health care needs, available services and health insurance.
Typically, patients are discharged before lunch, however, you will not be discharged until your providers complete the necessary medical records.
If your discharge time will be later, your provider or nurse will discuss this with you.
